Processing of fat tissue to isolate different stem/ progenitor cell populations is very hot topic right now. It’s hot and promising, because the first clinical trials, used adipose-derived stem/ progenitor cells were started. Also, because there are few competing companies, developing fat tissue processing devices for clinical use. Despite excitement and impressive clinical translation, there are many questions and technical issues, associated with this approach. For example:

  • What kind and how many different stem/ progenitor populations could be isolated from human fat tissue?
  • How can we identify them and distinguish based on markers?
  • What is the best way to process fat tissue in order to enrich progenitor populations before sorting?
  • What flow cytometry parameters will allow us a clear dissection of populations? What gating strategy should be?
  • Could different protocols from different labs, dissect the same functional progenitor cell population?

The in vivo progenitor of culture-expanded mesenchymal-like adipose-derived stem cells (ADSC) remains elusive, owing in part to the complex organization of stromal cells surrounding the small vessels, and the rapidity with which adipose stromal vascular cells adopt a mesenchymal phenotype in vitro. Immunohistostaining of intact adipose tissue was used to identify three markers (CD31, CD34, and CD146), which together unambiguously discriminate histologically distinct inner and outer rings of vessel-associated stromal cells, as well as capillary and small vessel endothelial cells. These markers were used in multiparameter flow cytometry in conjunction with stem/progenitor markers (CD90 and CD117) to further characterize stromal vascular fraction (SVF) subpopulations.
